We were able to determine this is a 3D Robotics 3DR0654 Tarot T-2D Brushless Gimbal Kit for 3DR IRIS+/Quad/Y6/X8+ by the part number located on the factory original packaging.

The two-axis Tarot Brushless Gimbal pre-calibrated and configurated for 3DR IRIS+ uses cutting-edge gyroscope technology to ensure you get crisp, stable images from your 3DR drone. Designed to accommodate the GoPro HERO 3 and HERO 4 BLACK. The T-2D offers manual camera control from your controller’s Tilt knob, but the model you get from 3DR is also specially configured to allow the autopilot to control the camera automatically, which is critical to using applications like 3PV Follow Me mode on IRIS+. This T-2D ships pre-assembled by 3DR, and includes the mounting plate and hardware for your IRIS+, Y6, and X8.
